Pharmacist Partners has developed a pharmacist-led, clinical service that can be provided to clients (patients, their caregivers or proxies) of all non-medical, home-care support agencies/services/companies.

One of the chief contributors to hospital re-admission and long-term care/nursing home admissions is the inability of aging adults to effectively manage the multiple medications they take to treat chronic conditions. Pharmacist Partners’ medication management program will enable seniors to age in place — at a fraction of the cost of living at an inpatient facility.

The benefit to the non-medical, home-care support companies is keeping their clients at home longer, thereby increasing the length of time they can provide services to this population.
